Immunization Records

Records

Recently had dinner with close friends that got married 2 years ago, and are headed to Laos to teach English. They are getting all their Immunizations in order to be able to make the journey. The wife brought to my attention what hassle it is to get a name change and with that how the system didn’t automatically switch things. My FiancĂ© will go through the same trouble but with this new knowledge we have a few questions to start with.

Let’s get you Started…

  • Is the information found with this previous name?
  • Can I get a copy of the updated Immunization record?
  • Along with my visit are there any immunizations that we can update while I am there?
  • What immunizations am I missing?

Your local health department can give you a copy of your Immunization record.

Things are going fast. There might not be anytime to ask questions in the front in because time-sensitive treatments may occur. Remember be kind.

Here is a start…

  • What type of stoke is it?
  • What was the cause?
  • What are the treatment options?
  • What are the outcomes from this?
  • How can I help my loved one while I am in the room?
  • What new medicines will they be on during their stay? and what will they do?

Empowerment by Questions

What do I even ask? Having a list of good questions to ask the medical providers when your loved ones are at the hospital gives you a better chance in the advocacy of your loved ones. Things won’t be easy and this is not a thing that will make things go away. The hope is to get you started by having sets of questions. We will go with a weekly series and by common cases.

Let’s start with…

“Have a place to write and record”

  • What is wrong with my loved one?
  • What does this mean?
  • What is the next step?
  • How many Doctors are taking care of my loved one?
  • What is their name and speciality?
  • Which physician is considered the Primary Doctor?
